CALIBRATE ADHD PROTOCOL
Psychological assessment, including this ADHD evaluation involves the administration of standardized instruments/tests. In this case, we will administer the QB Check, the Beck Depression Inventory, the Beck Anxiety Inventory, the Adult ADHD Self Report Questionnaire, and a clinical interview. The results provide information about you that may be commensurate with your own observations and worldview, or may be quite different than preconceptions. We will discuss the results and recommendations following administration of this assessment so they can be helpful to you. As the client, you and you alone hold the right to disclose the results of your assessments to whomever you choose through written consent which you will be able to provide after checkout.

NATURE OF THE COACHING RELATIONSHIP

Coaching is a process in which the client wishes to achieve personal growth in their life and the coach is there to act as a guide and to provide structure and method to the process of analyzing the challenge and aiding the client in developing strategies and ways of thinking to accomplish this change. It is the client who achieves change, the coach can not ever do this for the client.

Coaching can involve any aspect of the client’s life, including mental, emotional, physical, relational and financial. The client retains full responsibility for all of these, their own wellbeing and is solely responsible for any actions or decisions they take as a result of the coaching process. The parties agree that the coach is not and can not be held liable for any action or inaction or any direct or indirect result of the coaching process. The client recognizes that coaching cannot ‘cure’ anything, mental or physical and is not a replacement for therapy or medical help. The client recognizes that no part of this program is medical or therapeutic advice and should not be treated as such.

CONFIDENTIALITY

All terms and conditions of this Contract and any materials provided during the term of the Contract must be kept confidential by the Client, unless the disclosure is required pursuant to process of law Disclosing or using information from this Contract or coaching sessions for any purpose beyond the scope of this Contract is expressly forbidden without prior consent of the Coach.

RECORD KEEPING AND DATA PROTECTION

The coach may take notes in sessions (or record sessions) in order to better understand the client and aid in preparation for subsequent sessions. These notes will never be passed on to a third party. Any notes will be retained for the period of coaching, and then a period of 3 years, in case the client wishes to return to coaching at a later date. Within our strict code of confidentiality, the coach will need to keep your name, contact details, and attendance data on record in office. The coach keeps a record of all appointments offered, attended, and canceled, and of the payments made. Subject to local regulation, you have a right to see all of your notes and records. If you would like to see the information that the coach may have about you, please ask at any point throughout the coaching process. In accepting the offer of coaching you agree to these records being kept.

SCHEDULING AND CANCELLATIONS

Group Coaching calls will be scheduled at various times in order to meet most people's schedules. Attendance in group coaching is not a requirement for this program. We will have individual calls as part of the program. Once these are scheduled, rescheduling arrangements can be made at any time with at least 72 hours notice prior to an arranged session. If the Coach needs to reschedule an appointment for any reason, they reserve the right to do so, and will act in good faith to ensure all coaching sessions will be had before the termination of this Contract.

Any CALIBRATE events held by the coach will be free for the client to attend. The coach will do their best to give proper notice of dates for special events. In the case the client is unable to attend special events, a recording of the event will be provided.

GUARANTEE

If the client fulfills the assignments throughout the duration of this contract, but their baseline behaviors are unchanged as defined by these trackers by the termination of this contract, they will receive an additional month of group coaching for free.

If at any time problems arise with the coaching process, the client and/or coach are responsible for addressing these issues and working together to resolve said problems to the best of their ability and in good faith to the satisfaction of both parties.

CONSENT TO RECORD

The client consents to the recording and distribution of group sessions held throughout the duration of their program. Videos will be shared with the Calibrate Community. Client wins and breakthroughs may be used as program testimonials in advertisements, websites, and social media sites. Any information shared will maintain confidentiality as it pertains to this program and the Coach will seek permission prior to the use of said testimonials.

EXCLUSIVITY

The Parties agree that this Contract is not an exclusive arrangement and that the Coach is entitled to enter into other similar agreements with other clients.

INTELLECTUAL PROPERTY

Hereby, the Client agrees that any intellectual property provided to him/her/they by the Coach will remain the sole property of the Coach, including, but not limited to, copyrights, patents, trade secret rights, and other intellectual property rights associated with any ideas, concepts, techniques, inventions, processes, works of authorship, confidential information or trade secrets

THE UNDERTAKING OF THE CLIENT

I am motivated and committed to the coaching process. I understand that in order to achieve results, my full participation and commitment will be required. I accept full responsibility for myself and any actions I take that might result from Coaching and I promise that I am healthy enough physically and mentally to engage in coaching. I know that I must take responsibility for my life in order to empower myself to achieve the change I wish to make and that no one else can do this for me. I agree to honor my scheduled session times, and fulfill my payment obligations.

I have read and agree with the Coaching Contract Agreement.
